Donald Trump — subject of the public physician-memo record (official portrait).

The AI Files · Trump’s public medical record — campaign physician letters, White House physician memos, COVID hospitalization, chronic venous insufficiency, aspirin bruising, cognitive-screen scores, age-at-inauguration politics, and the false 2025 death rumor (2015–2026) · 2015–2026

Health, age and the death rumor

In May 2026 the White House physician’s memo said President Trump was in “excellent health,” scored 30/30 on a cognitive screen — and also told him to lose weight, move more, and prefer low-dose aspirin.

That tension — upbeat fitness language beside ordinary aging advice, beside viral claims he was dead or secretly dying — is the whole file.

Chapter 1

The letter that sounded like the patient

In December 2015 the Trump campaign released a physician letter that read like a rally speech: blood pressure “astonishingly excellent,” stamina “extraordinary,” and a promise that Donald Trump would be “the healthiest individual ever elected to the presidency.” [1][2] It was short on labs and long on adjectives. Reporters noticed. Supporters waved it anyway.

Chapter 2

A second letter with numbers

September 2016 brought a more conventional page from the same doctor, Harold Bornstein: cholesterol figures, blood pressure, weight 236 pounds, rosuvastatin and low-dose aspirin, “excellent physical health.” [3] The tone cooled. The political job was the same — show a chart-shaped object before an election.

Chapter 3

“He dictated that whole letter”

In May 2018 Bornstein told CNN the hyperbolic 2015 letter had been dictated by Trump himself. [4][5] That admission does not automatically falsify every later lab line. It does force anyone honest to stop treating campaign letterhead as peer-reviewed medicine. The dictation entry sits on the critical path for that reason.

Chapter 4

Walter Reed, January 2018

As president, Trump’s first showcase physical ran through the White House Medical Unit at Walter Reed. Rear Admiral Ronny Jackson’s memo — height 75 inches, weight 239, blood pressure 122/74, “overall health is excellent,” diet and exercise counseling, a statin increase — was read aloud in the briefing room. [6][7] Advocates had just urged a neurological screen; the politics of cognition were already in the building. [8]

Chapter 5

COVID, oxygen, steroids

October 2020 was not a memo about cholesterol. Trump went to Walter Reed with COVID-19. Physician briefings, as reported contemporaneously, described transient oxygen drops and dexamethasone among other treatments before discharge. [9][10][11] Camps still argue about adjectives — “severe” versus “he walked out.” The hospitalization itself is not a rumor.

Chapter 6

Age becomes the weather

Out of office and then back on the ballot, age stopped being a demographic footnote. By 2023–24 it was climate. Age politics did not need a new lab value to dominate cable. It needed rivals, clips, and fear of decline.

Chapter 7

The oldest oath

On January 20, 2025, Trump took the oath again — the oldest person ever inaugurated as president of the United States. [12][13][14] That is a calendar fact. It is not, by itself, a diagnosis. It is also not nothing. Every later fitness fight happens in its shadow.

Chapter 8

April 2025: 224 pounds, 30/30

Capt. Sean Barbabella’s April 2025 annual physical memo — hosted as primary text by the American Presidency Project and as a PDF by the New York Times — reported weight 224 pounds, a Montreal Cognitive Assessment score of 30 out of 30, and “fully fit” language. [15][16][17] Second-term disclosure had a baseline.

Chapter 9

Swollen ankles, bruised hands

Photos did what hearings often cannot: they forced a letter. On July 17, 2025, Barbabella’s memo said Doppler studies showed chronic venous insufficiency, a common condition after age seventy; no deep-vein thrombosis in that workup; labs and echo reassuring; hand bruising consistent with aspirin plus endless handshakes; “remains in excellent health.” [18][19][20] Viral clips that translated ankles into “stroke cover-up” were already ahead of the PDF.

Chapter 10

The weekend he “died”

Late August 2025, after a stretch with few public events, social platforms invented a corpse. PolitiFact’s reconstruction — labeled here as mainstream journalism, not a medical chart — traces a sliced JD Vance quote, schedule panic, and health-photo fear into #trumpisdead. [21][22] Getty golf photos and a Sept. 2 Oval Office appearance ended the death claim. [23][24] Trump called the ill-health rumor fake and said he had been active. Quiet is not a death certificate.

Chapter 11

“Nice, thin blood”

On New Year’s Day 2026, wires reported Trump telling the Wall Street Journal he takes 325 milligrams of aspirin daily — more than doctors recommend — to keep his blood “thin,” and that this explained the bruises. [25][26] Patient voice and physician preference parted on camera.

Chapter 12

May 2026: excellent, and lose weight

Three days after a May 26 Walter Reed exam, the White House released Barbabella’s annual memo in full: vitals, imaging language, a medication list, MoCA 30/30 again, ear scarring noted from a prior gunshot, residual leg swelling improved, bruising again tied to aspirin. [27] The summary said excellent health and full fitness for duty. The counseling paragraph said diet, low-dose aspirin, more activity, continued weight loss. [28][29] Weight had risen from 224 to 238 since April 2025. Both the adjective and the advice are in the primary text. Camps that remember only one clause are doing memory work, not reading.

Chapter 13

What a MoCA is — and isn’t

Perfect MoCA scores in released memos are real data points. [15][27] They are also brief screens. The MoCA entry refuses two lazinesses at once: treating 30/30 as a magical force field, and treating a rally clip as a differential diagnosis.

Chapter 14

The physician is on staff

Every modern White House Medical Unit release is an authorized summary by a physician who works for the president. [7][27] That structural fact feeds the “pure PR” theory. It does not license inventing a cancer the memo forgot to mention. Role conflict is a debate with evidence on both sides of *trust*; the memo text still has to be quoted first.

Chapter 15

Rival paths

Cover-up networks remove or recast the CVI and May memos and sometimes the Oval Office itself. Maximal perfect-health networks sand off counseling and the age record. Cognitive-collapse networks deny the ethics brake on afar diagnosis. Memo-cynics deny the Jackson/Barbabella class of documents. Psyop readers accept he lived and hunt a sponsor for the rumor. Double-standard readers turn the whole fight into media studies. Each card on this page shows what must be wrong for that path to win.

Chapter 16

Open unknowns

The private problem list, if any exists beyond the memos, is unknown. Day-to-day aspirin milligrams are only partly known. Full neuropsychological batteries are not public. Whether someone *organized* the death rumor beyond engagement farming is unknown. These are labeled unknowns — not prizes for the most cinematic guess.

Chapter 17

How memory moved

From “healthiest ever,” through Jackson’s excellent, through COVID scare, through the age primary, through ankle photos and a false funeral online, to a May memo that refuses to be only one thing — public memory did not travel in a straight line. the 2025–26 memory phase is where rumor forensics and physician PDFs finally shared a news cycle.

Chapter 18

What it changed

We now treat hand bruises as political medical events. We know a quiet weekend can mint a death trend. We still do not get raw charts. The Goldwater-rule argument never left. Oldest-oath arithmetic is permanently available to whoever wants a club or a shield.

Chapter 20

Read the memos

If you take one habit from this file, take this: when someone says “his doctor said,” open the memorandum. The fight worth having is over what the sentences mean — and what a free society still cannot see — not over a screenshot of someone else’s verdict.

Mainstream path: authorized memos + false death rumor

Where it started

White House physician practice plus wire and fact-check reconstructions of 2025 rumor.[1][3][4]

Why it lives

Matches the densest primary paper trail and the hard falsification of the death claim.

If it were true

Public debate should track memo text (including counseling) rather than TikTok differentials.

How it squares with the record

CVI and bruises are disclosed, not ignored. [2]

Death rumor collapsed on camera. [5]

Excellent + counseling coexist in May 2026. [1]

What holds up Documented

What new information would make it true
  • Continued consistency between public function and memo claims.
  • Any new release still fitting CVI/aspirin/weight narrative.
What would prove it false
  • Credible leak of a contradictory grave diagnosis with documentary proof.
  • Demonstration that cited memo texts were fabricated wholesale.

How this guide works

Sources and method

This page extracts the authorized public medical record — physician letters and White House physician memoranda, contemporaneous briefings, and well-sourced wires — then labels the mainstream path and scores rival major-network readings against a critical path.

Primary documents sit on the spines (Bornstein letters; Jackson 2018 PDF; Barbabella April 2025, July 2025, and May 2026 memos via White House / American Presidency Project / NYT document hosts). Secondary fact-checks (PolitiFact, Snopes, PBS) are labeled as mainstream reconstructions of the death rumor, not as substitutes for the physician memos.

We do *not* invent diagnoses. Bruising, venous insufficiency, weight, medications, and MoCA scores are reported as the memos state them. Armchair cognitive diagnosis from video is treated as a claim network, with medical-ethics context (Goldwater-rule debate), not as a clinical finding.

Hard stops: Ukraine war out of scope; never cite Grokipedia; no middle-layer trap (title the primary, don’t spine a hostile secondary’s verdict as if it were the memo).

Limits are structural: the public does not receive raw imaging files, full specialist notes, or an independent outside physician’s complete chart. Fitness-for-duty language in a White House physician memo is an authorized summary, not a peer-reviewed paper.
