The familiar figure of 600 combines two initial groups: 399 men with syphilis and 201 men without it. CDC identifies the original infected participants as men recruited with late-latent syphilis, drawing on the investigators’ 1936 publication. The comparison men were not assigned to be infected. These numbers describe enrollment, not six hundred infections caused by researchers. [1]
The distinction also matters when reading later statistics. The advisory panel found that some comparison participants subsequently acquired infection or had positive results on more specific tests that were unavailable at enrollment. Some were moved into the syphilitic group. Group membership and diagnostic technology therefore changed across the decades. A later table can legitimately contain numbers that differ from a summary of the original cohorts; it needs a date and a definition. [2]
The study enrolled Black men aged twenty-five or older. Their exclusion from informed decision-making was not a technical error in bookkeeping. Investigators used a comparison group to pursue a scientific question while failing to meet obligations to the people supplying the observations. Correctly identifying who had infection at the beginning helps expose that actual failure. It offers no defense of withholding information or recommended care from either group.
