On October 31, 2025, Minnesota’s Medical Assistance Feeding Our Future Services program ended after CMS approved a state-plan amendment terminating the §1915(i) benefit. DHS said services could not be provided or reimbursed after that date. A CMS letter dated October 24, 2025, confirms approval with a November 1, 2025 effective date for termination.

DHS had flagged termination interest in August 2025 citing widespread fraud and said it could not wait to see whether new integrity tools would suffice. Public comments raised participant-impact concerns. Shuttering is program integrity — not a mass conviction.

On September 18, 2025, USAO-MN announced federal wire-fraud charges against eight defendants in the first wave of FOF fraud cases — Brilliant Minds/Foundation First, Faladcare, Leo Human Services, and Liberty Plus captions with multi-million-dollar claim tallies as alleged. Acting U.S. Attorney Joseph H. Thompson announced the wave; charging documents remain allegations until plea or verdict.

On March 24, 2025, Abdihakim Ali Ahmed pleaded guilty to wire fraud and money laundering in the Feeding Our Future charging class tied to ASA Limited LLC. The USAO package later records an August 26, 2026 sentence of 54 months before Judge Brasel. Ahmed Abdullahi Ghedi appears on the same ASA Limited charging geography — partnership adjacency is not automatic proof of identical knowledge on every claim. Indictment was accusation until that plea; Omar donor adjacency stays separately labeled and is not an invented co-conspirator finding.

A December 18, 2025 USAO packaging had charged the pair. Guilty pleas convert their lane; they do not convict first-wave defendants. Sentencing remains for the court.

In July 2020 Minnesota became the first state to offer Medicaid coverage for Feeding Our Future Services — consultation, transition, sustaining services, and moving expenses for seniors and people with disabilities including mental illness and substance-use disorders. (A September 18, 2025 USAO release briefly misdates the start as July 2022 while citing 2021 payouts; July 2020 matches DHS notices and later USAO packaging.)

By design the program had low barriers for providers and beneficiaries and minimal records requirements for reimbursement. USAO later said those features made FOF susceptible to fraud. Design vulnerability is not itself a crime and is not a free pass that erases charging papers.
